Every URL below was requested on September 4, 2026 and returned a 200 response, or a 200 after a redirect within the same government domain, in which case the final address is what we link. Six boards run bot protection that refuses automated requests; those are marked and were opened in a browser instead. Government sites move; if a link fails, the NABP directory in the sources is the fallback, and please tell us.

Three things every board can do for you

  1. Confirm a licence. Number, status, type, expiry, pharmacist in charge.
  2. Show discipline. Board orders, consent agreements, fines, probation, revocations. Usually a separate page from the lookup.
  3. Take a complaint. About a pharmacy located in the state, or a non-resident pharmacy shipping into it. This is separate from FDA MedWatch; file both when both apply.

Two things no board does

It does not approve drugs, and it does not rate pharmacies. An active licence is the starting condition, not an endorsement. For the federal layer, see who inspects a compounding pharmacy.
